Property owners, Homeowners Associations, business owners, and residents responsible for maintenance of a surface sand filter must submit the following form annually by December 31 to the Engineering Department (one form per filter). Please fill out carefully and hit submit when completed. You may upload photos and files to this form. You cannot save this form and return to it. Please call us if you have questions.
